We help screen for diabetes and identify individuals who may need follow-up care. Screenings catch early signs of diabetes before symptoms get worse. Early detection is crucial in managing the disease, preventing serious complications, and reducing long-term health risks.

We’ve partnered with KC Free Eye Clinic to help provide free vision screenings and glasses to elementary students.

Recognizing that mental health is an important aspect of a person’s well-being, we have been distributing mental health kits at community gatherings. These kits help reduce the stigma around seeking help and ensure that individuals have the tools they need to navigate life’s challenges.
Living with diabetes can be a daily challenge and it became even more difficult for Monica after she lost both their insurance and access to reliable transportation. Suddenly, managing her blood sugar felt impossible. Thankfully, our Community Health team was there to help. We not only provided a free A1c check, but we also connected Monica with our partners at Care Beyond the Boulevard, where she can now receive free medical care without worrying about how to get there. It’s a powerful reminder of how a single point of connection can lead to lasting stability.
During one Community Health event our team used our diabetes screening testing to identify a man in his 20s with dangerously high A1c levels. Because our team already had established partnerships with local healthcare providers, they were able to immediately connect him to the necessary care, preventing potentially severe complications.
Once unhoused and struggling with mobility due to illness, Sharon was heartbroken to part with her beloved dog. Thanks to the work of our Community Health program, we were able to provide equipment to her that restored her independence. The moment she received it, Sharon wasted no time in returning to the shelter to bring her furry friend home.
Community Health Manager
Kealy is one of the original members who helped establish our Community Health program in the aftermath of COVID-19. She later served as a Public Health Analyst for the Health Resources and Services Administration (HRSA), where she oversaw federal health surveillance for 10 states and led HRSA’s maternal and rural health efforts. She has returned to Heart to Heart International to serve as Community Health Manager where her proven dedication and diverse background make her an exceptional advocate for under-resourced communities and a vital asset to our team.
