I hereby grant and convey unto HEART TO HEART INTERNATIONAL all right, title and interest in any and all photographic images and video/audio recordings made by HEART TO HEART INTERNATIONAL during my volunteer time with HEART TO HEART INTERNATIONAL, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. I understand that if I sign up on behalf of other participants, I have received consent from them to agree to these terms on their behalf. Participants under 18 must have these terms agreed to by a parent/guardian.

I understand and agree as a HEART TO HEART INTERNATIONAL volunteer I assume all risks of loss or injury while participating in the volunteer program. I understand that if I sign up on behalf of other participants, I have received consent from them to agree to these terms on their behalf.
